Add/Edit a Survey

Version 14.19

Overview

This section will introduce you to the section of the Preventive Exam where you can document surveys. You will be guided through the fields to easily review and document information.

Note: If surveys are submitted as tasks to be completed from the Tracking window, open the Task and proceed to Step 5.

Add a Survey

  1. From an open well visit note, click Surveys. The survey window lists all prior surveys.
  2. To add a new survey, click the New button
  3. Click a folder located in the Categories panel. A list of surveys is displayed.
The folders listed in the Categories panel may be different if the practice has customized.

  1. Double-click the survey or highlight a survey and click the Select button
  2. Click the drop-down menu in the Informant field and select from the list
  3. To restrict who can view the survey, click the drop-down menu in the Visibility field and select from the list. The default is Any staff member.
  4. (Optional) To add a diagnosis to the survey, click the DX field and select from the list.
  5. (Optional) To add an assessment code, click into the Assessment field and begin entering text or click the drop-down menu and search.
Some surveys may have the Assessment field populated with the Diagnosis code used for billing. 
  1. The Admin by field defaults to the user completing the survey, to update it, click the drop-down menu and select from the list.
  2. The Ordered by field defaults to the provider ordering the survey, to update it, click the drop-down menu and select from the list.
  3. Answer the questions associated with the selected survey.
  4. Click the Save button.

Edit Survey Answers

  1. From an open well visit note, click Surveys. The survey window is displayed listing all prior surveys.
  2. Highlight the survey to edit and click the Edit button .
  3. Select the question in the bottom panel, click the drop-down menu in the Answer field, and select from the list.

  1. Click the Save button .
  2. (Optional) If the survey was scored, click the Rescore button.
  3. Additional or optional fields that can be changed.
  • Assessment: Click the drop-down menu and select from the list.
  • Status: Click the drop-down menu and select from the list.
Note: If a CPT code is attached to the survey, the status must be set to Informed or Complete to pass to the electronic superbill.
  • Notes: Type additional information in the Notes field related to the complete survey.
  • DX Description: Click the field and click the drop-down menu to change or add a diagnosis code.
  • Visibility: Click the drop-down menu and select from the list to change the current visibility level.
  1. If there are any changes, click the Save button.

  1. Click the Patient Chart button
  2. Search for and select the patient.
  3. Click the Well Visits tab
  4. Click the Start a New Note button
  5. Click the Surveys tab. The survey window is displayed and lists all prior surveys.

  1. To add a new survey, click the Add Survey button . The Survey Forms Explorer window is displayed.

  1. Click a folder located in the Categories panel. A list of surveys is displayed.
The folders listed in the Categories panel may be different if the practice has customized.

  1. Double-click the survey or highlight a survey and click the Select button.
  2. Complete the following information:
  • Informant: Click the drop-down menu and select from the list.
  • (Optional) Visibility: Click the drop-down menu and select from the list if the survey is restricted to a visibility level.
  • Admin by: Will default to the user completing the survey, to change it click the drop-down menu and select from the list.
  • Ordered by: Will default to the provider ordering the survey. If not ordered by a provider click the drop-down menu and select from the list.

  1. Answer the questions associated with the selected survey.
  2. Click the Save button.

Edit Survey Answers

  1. Highlight the survey to make changes from the Survey tab.
  2. Click the Edit button.
  3. Select the question and click the drop-down menu in the Answer field.

  1. Click the Save Changes button  or Cancel Changes button  to discard changes to the survey.
  2. If the survey was scored, click the Rescore button.

  1. Additional or optional fields that can be changed:
  • Assessment: Click the drop-down menu and select from the list.
  • Notes: Enter additional information in the Notes field related to the complete survey.
  • DX/DX Description: Click the field and select from the drop-down menu to change or add a diagnosis code.
  • Visibility: Click the drop-down menu to change the current visibility level.

  1. Click the Save button.
