The list of newborn hospital facilities is populated in the Registry Look up Table:
Path: Practice Management tab > More button (in the Reference Data group) > Vaccine Registry Codes
The steps below will guide you on entering surrounding hospitals where a vast amount of your patient population is born so that this can be recorded in the patient's birth history record.

Entering Birth Facilities
- Select Birth Facility from the Code table to review/edit drop-down field.
- Check the Allow editing checkbox. A warning window appears:
- Click OK.
- At the bottom of the window, click the Add button . A blank entry line appears in the grid.
- In the Code field, enter the Birth Facility's official state or federal code.
Note: This Code should be obtained directly from the facility prior to the information being entered into the Immunization Registry Codes window. |
- In the Description field, enter the name of the birth facility.
- In the Sort Order field, enter a sort number.
- Click the Save button .

Entering Birth Facilities
- Select Birth Facility from the Code table to review/edit drop-down.
- Check off the Allow editing check box.
- Click OK to the warning.
- At the bottom of the window click the Insert the record button .
- Enter the Birth Facility official state or federal code. Note: Contact the facility to obtain codes before entering them into OP.
- Enter the Facility Name.
- Enter a Sort number.
- Click the Save button .
