CHADIS - Sending Surveys to the Portal

Overview

The information on this page describes the process a practice will follow when using CHADIS surveys and Patient Portal. 

  1. When the appointment is confirmed, the parent can access the survey on the Patient Portal. 
Note: The CHADIS survey is available on the Patient Portal when the confirmation status for the patient appointment reads Confirmed, Left message, or Self-confirmed. The patient appointment confirmation must be done, at a minimum, one day prior to the appointment. It is also important that appointments are scheduled using templates with CHADIS surveys attached.

  1. The parent taking a CHADIS survey for the first time must accept the CHADIS Terms and Conditions of Access.

  1. The survey will then be available for the parent to take.

  1. Take the survey.

  1. Once completed, the survey can be submitted. Select the SEND NOW button to submit the survey.

  1. When the survey is submitted, the parent will receive an acknowledgement on the screen for completing survey.

Once a survey has been started by a user, it must be completed by that same user. CHADIS will not allow a different user to complete a survey partially completed by another respondent.
