Change Request Therapeutic Interchange

Overview

Therapeutic Interchange messages requests are sent by a pharmacy for any prescription request that can't be categorized as either a Generic Substitution or a Prior Authorization request. Pharmacist has received that prescription and may need clarification about the medication dosing amount, they may find a conflict and actually be suggesting a different medication completely which could be a viable alternative to the original prescription. It is the provider’s responsibility to review and make a decision, best practice is within 48 hours. For instruction on enabling Change Request in OP, click here.

Accept a Therapeutic Interchange Change Request

  1. Navigate to the Refill/Change Requests tab: Main Navigation Panel > e-Prescribe > Refill/Change Requests tab.
  2. Double-click on the medication request for a Therapeutic Interchange or highlight the medication and click the Accept/Deny button.
  3. If the patient has medication allergies a Warning box will display, click the OK button.
  4. The eRx Change Summary window opens. Review the pharmacy changes in the As Dispensed Change Request panel.
  5. Click the Accept button. The medication is removed from the Refill/Change Requests tab.

  1. Review the Delivered tab to confirm the new medication was sent to the pharmacy.
With any Surescripts message either inbound or outbound there is an opportunity for an error in the actual transmission of the message. Messages that “Error” are no longer valid and therefore the Pharmacy may need to be called or a new prescription written.

Accept a Therapeutic Interchange with Changes

  1. Navigate to the Refill/Change Requests tab: Main Navigation Panel > e-Prescribe > Refill/Change Requests tab.
  2. Double-click on the medication request for a Therapeutic Interchange or highlight the medication and click the Accept/Deny button.
  3. If the patient has medication allergies a Warning box will display, click the OK button.
  4. The eRx Change Summary window opens. Review the pharmacy changes in the As Dispensed Change Request panel.
  5. Click the Accept With Changes button. If the patient has medication allergies a Warning box will display, click the OK button.

  1. Make changes to the prescription and click the Save button. The medication is removed from the Refill/Change Requests tab.
  2. Review the Delivered tab to confirm the new medication was sent to the pharmacy.

Deny a Therapeutic Interchange Change Request

  1. Navigate to the Refill/Change Requests tab: Main Navigation Panel > e-Prescribe > Refill/Change Requests tab.
  2. Double-click on the medication request for a Therapeutic Interchange or highlight the medication and click the Accept/Deny button.
  3. If the patient has medication allergies a Warning box will display, click the OK button.
  4. Click the drop-down and select a Denial reason.
  5. Click the Deny button. 
  6. All Therapeutic Interchange denials will appear in the Refills/Changes Denied tab.
