About
The Mental Health Monitor (or Tracker) is a dashboard located in Clinical Analytics that leverages internal English surveys: GAD-7 and Depression Screen: PHQ-9: Modified for Teens. If your Practice has modified either of these two surveys or has named them differently, you will need to download the new versions from the Survey Downloads article. CHADIS surveys will not be utilized at this time.
This Dashboard is sectioned into 5 cards that display the patient's data broken down by focus.
Card 1: Patient List
This view shows all of the patients with a Problem List entry of Mental Health Promotion. It provides a link to their chart, and displays their date of birth, their most recent Sick and Well Visits, and their next scheduled appointment, if applicable, with color coding to indicate if it has been greater than 30, 60, or 90 days since the patient was last seen. This will help you make sure that appropriate, timely follow-up is provided for these patients.
- Date Range: N/A in this card.
- Name: Patients who have qualified by the Problem List entry of Mental Health Promotion. This is a clickable link to open the patient chart.
- Last Well Visit: The date of the patient's last Well Visit. If blank, there is no Well Visit documented. Below is the indicator color coding:
No indicator = The visit was within the last 30 days from the current date.
= The visit was within 31-60 days from the current date.
= The visit was 61-90 days from the current date.
= The visit was over 90 days from the current date.
- Last Sick Visit: The date of the patient's last Encounter (Sick) Visit. If blank, there is no Encounter Visit documented. Below is the indicator color coding:
No indicator = The visit was within the last 30 days from the current date.
= The visit was within 31-60 days from the current date.
= The visit was within 61-90 days from the current date.
= The visit was over 90 days from the current date.
- Next Scheduled Appointments: The date of the patient's next scheduled appointment. If blank, no appointment is scheduled.
- Status: The status of the Mental Health Promotion Problem List entry. The statuses that are displayed in this column are: Active, Tracking or Resolved.
- Export: Gives you the option to export the Patient List. For instructions on exporting, see Clinical Analytics: Exporting.
- Maximize: Gives you the option to maximize the Patient List. To go back to all cards, click the button to restore the window.
Card 2: Outcomes
This view shows patients who are listed on the Patient List that have two completed PHQ9s: Modified for Teens or GAD-7s so that a comparison can be made between a prior and a recent score. For a patient to display in this view, the Practice must use the OP Internal surveys of GAD-7 and Depression Screen: PHQ-9: Modified for Teens. If the Practice uses CHADIS surveys, those patients will not be included in this view.
- Date Range: Allows you to choose from 30, 60, or 90 days from the current date or to enter a Custom date range from when the most recent survey was completed.
- Outcomes: Gives you the option to view patients by the PHQ9 or GAD7 survey.
- Outcome Measures List: This list is driven by the Outcomes radio button selected. Within the Outcome Measures List, you can click the patient name link to open the Patient Chart and view scores for the prior and recent surveys.
- Outcome Measures Pie Chart: This is a pie chart representation of the patients in the Outcome Measures List. You can click to select a single section of the pie chart or click the Multi-Select buttonto select multiple sections of the pie chart. Once selected, the Outcome Measures List is populated with the patients from the section(s). Refer to the Thresholds table below for the sections of the pie chart.
- Non Responders: Patients whose prior and recent scores fall within the same severity.
- Getting Worse: Patients whose recent score has increased and moved them to a higher severity.
- Responders: Patients whose recent score has decreased and moved them to a lower severity.

Card 3: Suicidal Ideation (SI) Risk
This view shows patients that have answered Yes to questions 12 or 13 or Several days or higher for question 9 on the Depression Screen: PHQ9: Modified for Teens survey. In order for scores to display in the Recent/Prior PHQ9 Score or Recent/Prior GAD7 Score fields, the patient must have two surveys completed.
- Date Range: Allows you to choose from 30, 60, or 90 days from the current date or to enter a Custom date range from when the most recent survey was completed.
- Name: List of patients that have at least one survey saved to their record and have answered one of the questions positively (12, 13 or 9) for the Depression Screen: PHQ9: Modified for Teens. This is a clickable link to open the patient's chart.
- Recent/Prior PHQ9 Scores: Scores are displayed if two surveys have been saved in the patient record. If blank, only one survey was completed.
- Recent/Prior GAD7 Scores: Scores are displayed if two surveys are saved in the patient record. If blank, only one survey was completed.
- Export: Gives you the option to export the list. For instructions on exporting, see Clinical Analytics: Exporting.
- Maximize: Gives you the option to maximize the Suicidal Ideation card. To go back to all cards, click the button to restore the window.
Card 4: Referral Follow Up
This view shows patients that are on the Patient List with Pending referrals by the selection made in the Date Range section.
- Date Range: Allows you to choose between the current date to 30, 60, or 90 days, or enter a Custom date range from the flag date of the Pending referral.
- Name: Patients who have qualified by the Problem List entry of Mental Health Promotion and have a Pending referral within the Date Range selection. This is a clickable link to open the patient's chart.
- Date: The date the referral was written.
- Referral Note: Name of the specialist the referral was written to.
- Diagnosis: The primary diagnosis code (DX 1) as entered on the referral.
Card 5: Survey Score Trends
This card displays a visual representation of the average score for each of the surveys on a weekly basis based on the date completed. You can hover over a data point to view the average for that week.
- Export: Gives you the option to export the list. For instructions on exporting, see Clinical Analytics: Exporting.
