Demographic Analysis and Recall: Care Plan Recall

Prior to running the Care Plan Recall Report, you will first need to run the Patient Demographics Report. The Care Plan Recall Report will display a list and/or recall patients due for items associated with a Care Plan.

Care Plan Recall Criteria

To conduct a for a Care Plan Recall, enter the following criteria before clicking the Search button:

  • Choose Recall plan(s) from the dropdown.
  • Choose a Recall item from the dropdown that is associated with the Recall plan.
  • Select the Co-morbid checkbox to include only patients who are enrolled in all selected Care Plans.
    or
    Select the Include if due for any item in plan checkbox.
  • Select the Include all qualified checkbox to include patients who qualify for the specified Care Plan that have not yet been enrolled.
Note: If you do not wish to include a particular patient or multiple patients in your report output or message file, select the Exclude checkbox next to the patient's name. For more information on the report output options, click here
  • Recall Date: This date field will default with the current date, but it can be set to a future date to proactively run Care Plan Recalls.
