About Encounter Questions (Exam, ROS, History)

Path: Clinical tab > More button (Customize group) > Encounter Questions button 

The Encounter Questions window builds custom descriptions and questions to add to the Exam and to the Review of Systems. This will then allow you to add these global questions to templates.

You can also update the History questions within the Past Medical History, Social History, Newborn History, Maternal Perinatal History and Abbreviated History. The Social History and the Non-Medication Allergies should never be edited.

Anything that is edited here will be available to use in the Well and Encounter templates.

QuestionsThe questions that are linked to a system can be added in this area. Questions can be added, removed, retired, edited, and rearranged.  Questions that are retired can be brought back at a later date.  Questions that are deleted will need to be rebuilt from scratch.

Add a System to a Section

To add a system to a section click on the in the middle of the page. Name your new system and choose the section that this will apply to. You can use the blue arrows in the middle of the page to change the sort order.

Add a Question to a System

To add a question to a system click on the on the lower half of the page. Type in your question, enter in a sort order and enter in the sex that this question is for. To add another question click on the , if you are done click on the  to save your changes.

Retire Questions

You may retire questions that the Practice will not use by clicking once on the question and then clicking on the button. Please keep in mind that any questions that you retire may be brought back in the future but any questions that you delete will need to be built from scratch. It is always suggested to retire questions instead of deleting them.

ROS Group Exam Types

ROS group has two Exam types: All Visit Types and Well Visit Only.

Edit History Groups

The following History Groups may be edited: Past Medical, Social, Newborn, Maternal Perinatal, Abbreviated.
Family Medical History cannot and should not be edited as this is built and harcoded to meet Meaningful Use. If any changes are made to this Group they will not be visible within the patient's chart and Family History.
