Office Practicum offers a Medical Home Report Bundle, which is a set of reports available in the QIC (Quality Improvement Calculator) Module. It is designed to assist practices in reporting measurable data related to Medical Home requirements. You may also browse the Medical Home Reports Help Page to learn more. If you would like more information on the Medical Home Reports Bundle, please complete the Medical Home Request Form and an OP Representative will contact you.

Concept TC
The video presentation discusses reporting for Concept TC. This includes the scoring for Core and Elective Criteria and the use of the QIC. Concept TC sets the practice commitment of transforming into a sustainable medical home. It has no reports in the QIC in OP. 
Concept KMThe video presentation discusses reporting for Concept KM. This includes the scoring for Core and Elective Criteria. During the video presentation you will be shown how to capture information in Office Practicum and use the QIC to track measurements. Concept KM is practice knowledge of your patients, caregivers, and community. You can use this knowledge to offer care that supports the population needs.
Concept ACThe video presentation discusses reporting for Concept AC. This includes the scoring for Core and Elective Criteria. During the video presentation you will be shown how to capture information in Office Practicum and use the QIC to track measurements. Concept AC centers around access and continuity of care established by the practice with consideration to needs and preferences of the practice population.
Concept CMThe video presentation discusses reporting for Concept CM. This includes the scoring for Core and Elective Criteria. During the video presentation you will be shown how to capture information in Office Practicum and use the QIC to track measurements. Concept CM centers around care management and support where the practice can identify patients that are high risk and effectively manage care.
Concept CCThe video presentation discusses reporting for Concept CC. This includes the scoring for Core and Elective Criteria. During the video presentation you will be shown how to capture information in Office Practicum and use the QIC to track measurements. Concept CC centers around care coordination and care transitions where the practice develops processes for tracking referrals, tests and care transitions. In addition, criteria will be used to evaluate how effective the practice is communicating with specialist and other providers.
