The Template Catalogs list templates that can be downloaded from the Help Center. Each link in the table takes you to a details page for the template, which includes the step-by-step instructions for downloading the template and the link to the downloadable template file.

Tip: OP strongly recommends that each practice that imports shared templates into their OP system carefully review the contents of those templates and customize those templates before using them in their daily workflows. Failure to do so may result in incorrect data (such as ICD settings in the template) that may be very difficult to correct. OP is not obligated to fix your templates or your data if you import or share templates and damage either your templates or data in the process.

Telehealth Templates

Telehealth Templates have been made available for OP practices to import and use. The templates are all named with TM- in the title to indicate they are useful for Telehealth (Telemedicine). It is important if you import them, that you warn all users when they search by other names, these will be included and should only be used for Telehealth visits. They have abbreviated exams and language in the Plan that are only appropriate when using Telehealth.

ASTHMA EXACERBATION: Follow up	This template is used by providers to document a follow up visit for asthma flare. Included on this template are the following standing orders: Update Asthma Action Plan, Pulse Ox, Peak Flow, and schedule a follow up visit in 3 months. Also included is an alternate order to evaluate and teach inhaler use. Medications are available on the Medications tab for prescribing.	1.0	7/24/2018

ASTHMA EXACERBATION: Moderate	The template is used by providers to document an asthma flare. Included on this template is the administration of Albuterol neb therapy in the office. Orders included are: Nebulizer treatment, pulse ox pre and post treatment, Nebulizer teaching, and repeat of nebulizer therapy. Automatic codes passing to the electronic superbill are for the Nebulizer treatment and teaching and 2 units for the pulse ox.	1.0	7/24/2018
