Register Family - Application Usage

The Register Family Application functions in two different ways:

  • If the portal user has not previously registered with your practice, or they have created a portal account but have not yet registered their children as patients, they will be greeted with the application in "Register Family" mode.
  • If the portal user is already registered with your practice and creates a portal account that links up to an OP contact, or they have previously used the portal to register their children as patients, they will be greeted with the application in "Register New Patients" mode.

"Register Family" Mode

Patient portal users who have not previously registered with your practice will be greeted with this form the very first time they log in. They will use this form to register their entire family with your practice.

  1. Location - not shown in the screenshot, If you have more than one practice location defined in Office Practicum, the patient portal will give the user the option to register at one location versus the other(s).
  2. Parent #1 Contact Information - All the contact information for the primary point of contact for this family. This should generally be the person who will be using this account to access the patient portal. Below are some explanations of the more cryptic fields:
  • Relationship - The user's role in this particular family.
  • Lives with patients below? - Check this if all the patients being registered as part of this family reside with this contact.
  • Work Phone - The first box is the main phone number, and the second box for an extension, if applicable.
  • Cell Phone - Cell number, followed by the Wireless Carrier's name.
  1. Parent #1 How Should We Reach You? - These four drop-down boxes collect information on how the primary contact would like to be notified for certain issues. In this version of the OP Portal, there is no real functionality behind this feature, but it is intended to be used in future versions for messaging purposes.
  2. Parent #2 Contact Information - Same purpose as the first Contact's, but intended for setting up a secondary contact with equal or lesser portal access. In this part of the form, there's an extra checkbox above the address information labeledCheck here if same address as above, which will make both points of contact use the same address and home phone number.
  3. Parent #2 How Should We Reach You? - Same purpose as the "How Should We Reach You" for the first Contact's, but may be configured differently for Contact #2.
  4. Insurance Information - Allows your portal users to file their insurance information for immediate use in the office.
  • Insurance company - Allows the user to choose from your practice's supported insurance companies, which pulls from the list you already have defined in OP. If using a different insurance or paying out of pocket, the user normally chooses "SELF-PAY".
  • Group Number (if not self-pay) - The user's insurance identification number.
  • Provided through parent/guardian - Tells you whose name is the primary point of contact for insurance.

  1. Patient (child) Information - This area allows you to register patients with the practice, up to six on the initial registration. This is the portal-user version of Office Practicum's patient register. It collects very minimal information on patients, and will use the checkbox on one of the Contacts above to establish address info. This form is intended to collect all-known information on not just children, but unborn babies as well.
  • Ethnicity - The user can select one of the options below. This is collected for Meaningful Use.
  • Main & Other Language - Records the languages spoken by this patient. Required for Meaningful Use.
  • Race - The user can check off all applicable items. Collected for the purposes of ARRA funding and Meaningful Use standards. This area is intended to be supported much better in future versions, after the Meaningful Use standards have been finalized.

"Register New Patient" Mode


Patient portal users who previously registered with your practice will see this form. Register New Patient mode allows them to add new patients to their family, one at a time.

  1. New Patient Details - Basic information about the patient.
  2. Ethnicity & Race - Optional areas collected for reasons relating to ARRA and Meaningful Use. These features are intended to be better supported in newer portal versions.
  3. From the following list, please choose the [Contacts] to link with this new patient - The portal will pull other Contact information from OP that relates directly to this user, or to the children related to this user. Using the checkboxes in this table will give the user the ability to link this new patient to the people who show up in this list, determine the address(es) that this new patient resides at, and give other contacts access to this new patient's records.
