Patient Registration

After your family is affiliated with the practice, you may need to add patients (such as a newborn). To do this:

Note: If you do not have Sign In credentials, click Create Account and complete the Create Account information.
  1. Sign in to the Patient Portal.
  2. Click Patient Registration, the New Patient Details window is displayed.
When adding a new patient, enter as much information as possible in the form. At the bottom there is a table of related contacts you may want to connect with the patient. To link one of these contacts to the new patient, select the Link? checkbox and specify the role this contact has with the new patient.
  1. Click the Submit button.

Depending on the practice, new patients may be placed on the waiting list. If a patient is on a wait list the parent/guardian cannot use the Online Patient Services section of the site until after their first office visit.

Patient Registration

When adding a new patient, enter as much information as possible on the form. At the bottom there is a table of related contacts you may want to connect with the patient.To link one of these contacts to the new patient, select the Link? checkbox and specify the role this contact has with the new patient.

When finished, click the Submit button.

Depending on the practice, new patients may be placed on the waiting list. Waitlisted patients cannot use the Online Patient Services section of the site until after their first office visit.
