About Patient Chart: Consents and Directives

The Patient Chart is the central point of access to all areas of a patient's complete Medical record.  Use the Patient Chart to access and maintain patient records.  

The Consent/Directive Document window lists and modifies the consent forms and directives that are linked to a medical record.


Note:  Just like a paper chart, the Office Practicum chart, and other windows in Office Practicum, contain personal health information that should be kept confidential. You should treat all windows in Office Practicum (like F8 Chart) just like you would treat a paper record of patient health information, never leaving that record out in the open for others to see. More specifically, you should always close all windows on your screen and log out of Office Practicum when you are done at your workstation.

Mark Reviewed buttonThe Mark Reviewed button adds a log entry that a medical review of records was conducted.

Link Medical Record buttonThe Link Medical Record button locates and links a medical record to either a Diagnostic Test Order, Vaccine, Vaccine Indicator, Encounter, Problem List or Referral Letter.

Consent Record gridThe Consent/Directives grid lists the consent records for the patient.  Records can be added, removed, and modified.  The Document Type (created in the Consent Record Types utilities window) can be set here along with the begin date, end date, and consent mode.

Attach Document button
The Attach Document button opens the Search for Scanned items window.  It attaches a scanned item to the patient record.

Attached Form and Pages tabsThe Attached Form and Pages tab displays a thumbnail of the scanned document.

The Office Practicum Medicaid Meaningful Use Visit Calculator is a free tool published by Office Practicum that allows OP practices to determine whether they meet the statutory thresholds to qualify for MU funding.
