About the Patient Chart: Problem List

Path: Clinical tab > Patient Chart button > Problem List

The Patient Chart is the central point of access to all areas of a patient's complete Medical record. Use the Patient Chart to access and maintain patient records.  The Problems List window opens the Problem list to easily modify for viewing on the chart and note. Selecting a note in the list will open the Problem List window.


Note:  Just like a paper chart, the Office Practicum chart, and other windows in Office Practicum, contain personal health information that should be kept confidential. You should treat all windows in Office Practicum (like F8 Chart) just like you would treat a paper record of patient health information, never leaving that record out in the open for others to see. More specifically, you should always close all windows on your screen and log out of Office Practicum when you are done at your workstation.

  • Problem List: The Problem List tab contains a complete list of problems for a patient's present and past medical history.
  • Scanned Records:  The Scanned records tab displays a patient's scanned item with the label "Chart:" as the item type in its document properties.  This tab sorts images by date, category staff, etc.  It also can change the recipient directly from the scan window.  This tab does not include items link directly to notes on the Problem List tab.

No Problems buttonThe No Problems button provides an indicator that there are no active problems for the patient.

Function buttonsThe function buttons print, add, edit, remove, and mark a note as reviewed.

Problem List gridThe Problem List grid displays the problems and notes for the patient.  Notes are categorized by status (active, tracking, resolved).  Clicking the Active only checkbox will display notes that are not resolved.

Prioritize buttonsThe Prioritize buttons organize the problems in the Problem List grid.

Problem List buttonThe Problem List button opens the Problem List window in the patient's chart.
