About Patient Chart: Risk Assessment Tab

Path: Clinical, Practice Management, or Billing tab > Patient Chart button > Risk Assessment

Within the Risk Assessment List, you can choose common risk factors and use them in the care plans for your patients. This helps with using OP for Meaningful Use as well as with OP Care Plans.

NoteJust like a paper chart, the Office Practicum chart, and other windows in Office Practicum, contain personal health information that should be kept confidential. You should treat all windows in Office Practicum (like F8 Chart) just like you would treat a paper record of patient health information, never leaving that record out in the open for others to see. More specifically, you should always close all windows on your screen and log out of Office Practicum when you are done at your workstation.

Current Risk Factors tabThe Current Risk Factors tab displays risk factors based on the child's age.

Review Log tabThe Review Log tab displays entries with the name and the date and time that the Mark Reviewed button was selected.

Mark Reviewed buttonThe Mark Reviewed button affirms the status of all risk factors on the risk factor list and marks the risk factors as reviewed.

Show all risk factorsAt times you may wish to update a Risk Factor that is not currently displaying for a child due to their current age (i.e. Smoking Status prior to the age of 13).  The Show All Risk Factors checkbox displays a complete list of Risk Factors in OP.

Risk Assessment buttonThe Risk Assessment button opens the Risk Assessment window in the patient's chart.
