About Patient Chart: Surveys

The Patient Chart is the central point of access to all areas of a patient's complete Medical record.  Use the Patient Chart to access and maintain patient records.  

The Surveys window documents and maintains a patient’s preventive exam surveys.


Note:  Just like a paper chart, the Office Practicum chart, and other windows in Office Practicum, contain personal health information that should be kept confidential. You should treat all windows in Office Practicum (like F8 Chart) just like you would treat a paper record of patient health information, never leaving that record out in the open for others to see. More specifically, you should always close all windows on your screen and log out of Office Practicum when you are done at your workstation.

Survey List
The Survey List panel displays all the surveys that were given to the patient.  Once a survey is selected, the survey questions and answers appear in the Question List panel.

The Question List displays the questions and answers for the selected survey in the survey list.

The New button adds a survey to the survey list.  Clicking the button opens the Survey Forms Explorer.

Delete button
The Delete button deletes the selected survey from the Survey List.

Rescore button
The Rescore button rescores the survey.  If a survey status is set to reviewed, it cannot be rescored.
