About Patient Chart: Surveys

Path: Clinical, Practice Management, or Billing tab > Patient Chart button > Surveys

The Surveys window in the Patient Chart documents and maintains a patient’s Surveys.

1Surveys buttonThe Surveys button accesses the Surveys window in the patient's chart.
2Function buttons
  • New: opens the Survey Forms Explorer to add a Survey to the Survey List. 
  • Delete: deletes the selected Survey from the Survey List.
  • Rescore: rescores the Survey if the status is set to a status other than Reviewed.
3CHADIS buttonsThe CHADIS buttons are only available to Practices that have partnered with CHADIS.
  • Refresh: updates the CHADIS Surveys with available updated survey results.
  • CHADIS: takes the user to the CHADIS site for access to the patient's CHADIS history.
4Survey List panel
The Survey List panel displays all the Surveys that were given to the patient. When a Survey is selected, the questions and answers are displayed in the Question List panel.
5Question List
The Question List panel displays the questions and answers for the selected Survey.

About Patient Chart: Surveys

Path: Clinical, Practice Management, or Billing tab > Patient Chart button > Surveys

The Surveys window in the Patient Chart documents and maintains a patient’s surveys. 

Note: Just like a paper chart, the electronic chart in OP contains personal health information and should be kept protected and confidential. Always close all windows on your screen and log out of OP when you are not at your workstation.

1Surveys buttonThe Surveys button opens the Surveys window in the patient's chart.
2Function buttons
  • New: The New button adds a survey to the survey list.  Clicking the New button opens the Survey Forms Explorer.
  • Delete: The Delete button deletes the selected survey from the Survey List.
  • Rescore: The Rescore button rescores the survey.  If a survey status is set to reviewed, it cannot be rescored.
  • Print: The Print button prints the survey.
3CHADIS buttonsThe CHADIS buttons are only available to practices who have partnered with CHADIS.
  • Refresh: The Refresh button updates the CHADIS surveys with available updated survey results.
  • CHADIS: The CHADIS button takes the user to the CHADIS site for access to the patient's CHADIS history.
