The Patient Encounter helps to code and complete an encounter visit.  The encounter note encompasses both narrative and comprehensive formats. It also allows you to customize the layout of the note.  The note is added to the patient chart.

The Counseling/Coordination of Care (CoC) window displays any counseling and coordination of care notes.

Complete List buttonThe Complete List button provides a complete list of encounter templates. The Templates button lists the templates that are created, edited, and reviewed in the Encounter Templates Editor. Templates are pre-written scenarios you can use in an encounter to document common pediatric diagnoses. Templates include detailed descriptions for the usual pertinent positive and negative findings of a wide variety of illnesses, with a standard assessment, plan and ICD-10 coding. Included with your installation of Office Practicum are Pediatric specific templates.

Prior Notes buttonThe Prior Notes button lists the previous encounter notes and messages.

Counseling/Coordination of care Text boxes

The Counseling and Coordination of Care text boxes are where you can enter narrative note of the exam.  Phrase Construction can be used to assist with the creation of a counseling or coordination of care narrative.  The narrative can also be created using the Phrase Construction button.

Time Spent indications
The time spent in minutes for the encounter can be entered in the Time Spent field.  The amount of time should determine the coding level for the visit.  It is important to document the time spend on counseling or coordinating care in the appropriate visit tab and to check the radio button that indicates if the Counseling or Coordination of Care was a key factor (greater than than 50% of the visit time).  The Function buttons delete an entire encounter note, edit an encounter note, or save an encounter note.

Visibility
The Visibility dropdown selection determines who has permission to view the Counseling or Coordination of Care note. In the case where there is a critical sensitive matter (ex: abuse, high-profile) the note visibility can be set to an option higher than Any Staff Member.

Phrase Construction button
The Phrase Construction button opens the Phrase Construction window.  Phrase Construction assists with the creation of notes and other text-based descriptions.
