About Patient Encounter: Counseling (Couns)/Coordination of Care (CoC)

Path: Clinical, Practice Management, or Billing tab > Patient Chart button > Encounter > New or Open Note button > Counseling (Couns)/Coordination of Care (CoC)

The Counseling/Coordination of Care (CoC) window displays any counseling and coordination of care notes.

Couns/COCThe Couns/COC button opens the Counseling and Coordination of Care window in the patient's encounter note.

Encounter Function buttonsThe Function buttons delete an entire encounter note, edit an encounter note, or save an encounter note.

Flag as Incomplete checkboxThe Flag as Incomplete checkbox will mark an encounter as unfinished.

Visit Status and Room drop-downsThe Visit Status and Room drop-down menus allow the user to select the appropriate status and location of the patient. These selections update the Tracking and Appointment windows.

All TemplatesThe All Templates field is used to select an encounter template that was created or edited in the Encounter Template Editor.  Predefined information will appear in the encounter when a template is selected.

Complete List buttonThe Complete List button provides a complete list of encounter templates.  Included with your installation of Office Practicum are Pediatric specific templates.

Prior Notes buttonThe Prior Notes button lists the previous encounter notes and messages.

Counseling/Coordination of care text fields

The Counseling and Coordination of Care text boxes are where you can enter the narrative notes of the exam.  Phrase Construction can be used to assist with the creation of counseling or coordination of care narrative.  The narratives can also be created using the Phrase Construction button.

VisibilityThe Visibility dropdown selection determines who has permission to view the Counseling or Coordination of Care note. In the case where there is a critical sensitive matter (ex: abuse, high-profile) the note visibility can be set to an option higher than Any Staff Member.
10Discussed with drop-downThe Discussed with drop-down allows the user to select a name from the Address Book or the patient's Clinical Contacts. It can also be used as a free-text field. This information will be used in the E/M Coding calculation for MDM and is included in the Coord of Care section of the Visit Note when populated using the Address Book or Coord of Care buttons.
11Independent interpretation of tests checkboxThe independent interpretation of tests checkbox is used when a provider interprets a diagnostic test and comes to their own conclusion. This information will be used in the E/M Coding calculation for MDM and is included in the Coordination of Care section of the Visit Note Summary.
