About Patient Encounter: Risk Assessment

Path: Clinical, Practice Management, or Billing tab > Patient Chart button > Encounter > New or Open Note button > Risk Assess

Within the Risk Assessment List, you can choose common risk factors and use them in the care plans for your patients. This helps with using OP for Meaningful Use as well as with OP Care Plans. This section of the Encounter Note is chart-driven, meaning that the information populated here is pulled from and pushed back to the same section in the Patient Chart.

Risk AssessmentThe Risk Assessment button accesses the Risk Assessment window in the patient's Encounter Note.

Encounter Function buttonsThe Function buttons delete an entire Encounter Note, edit an Encounter Note, or save an Encounter Note.

Flag as Incomplete checkboxThe Flag as Incomplete checkbox marks an Encounter as unfinished.

Mark Reviewed buttonThe Mark Reviewed button affirms the status of all risk factors on the risk factor list and marks the risk factors as reviewed.

Show all risk factors checkboxThe Show All Risk Factors checkbox displays a complete list of Risk Factors in OP. At times you may wish to update a Risk Factor that is not currently displaying for a child due to their current age (i.e. Smoking Status prior to the age of 13).

Review Log tabThe Review Log tab displays entries with the name and the date and time that Mark Reviewed button was selected.

Current Risk Factors tabThe Current Risk Factors tab displays risk factors based on the child's age.

Visit Status and Room drop-downsThe Visit Status and Room drop-downs allow the user to select the appropriate status and location of the patient. These selections update the Tracking and Calendar Schedule views.
9Pertinent checkboxThe Pertinent checkbox marks the selected risk factor as pertinent at the time of visit. Any risk factor marked pertinent that was marked as a SDOH in the risk assessment editor will show in the "Risk Assess" portion of the Note. Selecting the checkbox will be used in the MDM coding calculation for Encounters.

The Patient Encounter helps to code and complete an encounter visit.  The encounter note encompasses both narrative and comprehensive formats. It also allows you to customize the layout of the note.  The note is added to the patient chart.

Within the Risk Assessment List, you can choose common risk factors and use them in the care plans for your patients. This helps with using OP for Meaningful Use as well as with OP Care Plans.
