About Patient Chart: Demographics

Path: Clinical, Practice Management or Billing tab > Patient Chart button > Basic Information

The patient's basic Demographic Information window contains the patient' name, address, phone number and other basic information. You can enter and edit the patient's demographic information in this window.

Using the patient demographics you can:

  • Link Families
  • Confirm a new patient needs to be registered
  • Register a new patient
  • Print a patient’s demographics
  • Copy a sibling’s register record
  • Add relatives or contacts to a patient’s register record
  • Add, edit, copy or validate insurance
  • Add miscellaneous notes
  • Add privacy restrictions
  • View and add consent forms
  • View and add to the patient’s coordination of care
  • Change the family address.

Search patient record barThe search patient record search bar locates a patient that has been previously entered into the system.
2Basic Information button
The Basic Information button opens the Demographics window in the patient's chart.

Function buttonsThe function buttons insert a patient record, delete a record, edit a record, post an edit, cancel an edit.

Copy From buttonThe Copy From button incorporates information from a sibling patient that is already in the system.  It opens the Patient Directory and displays a potential list of a patient’s siblings based on the last name.

Merge buttonThe Merge button merges two patients together.  All patient data (except contacts, register, history, and newborn history information) is copied from the current patient to another selected chart.  The current patient is marked as merged and should not be edited.

Print buttonThe Print button prints the patient information.

About the Patient Register: Patient Tab

Path: Smart Toolbar > Register button > Patient tab

Whether a new patient is starting with your practice or your practice is transitioning from paper to electronic records, creating a Patient Register record is the first step in registering a patient in your database. The Patient Register assigns each patient a unique patient ID number, and automatically creates a Patient Chart for the patient.

Using the patient register you can:

  • Link Families
  • Confirm a new patient needs to be registered
  • Register a new patient
  • Print a patient’s demographics
  • Copy a sibling’s register record
  • Add relatives or contacts to a patient’s register record
  • Add, edit, copy or validate insurance
  • Add miscellaneous notes
  • Add privacy restrictions
  • View and add consent forms
  • View and add to the patient’s coordination of care
  • Change the family address.

The Patient tab is a storehouse for patient demographic information.  The demographic information can be created, edited, and reviewed from this location.

Patient Preferred Contact MethodsThe Patient Preferred Contact Methods select the method by which the practice will notify the patient for a particular reason. Each contact method can be different based upon the available reasons for contact. Available reasons for contacting the patient are visible based upon if you are using the Patient Message Exchange (PMX) or OP Notify. Contact methods include:
  • No contact
  • Mail Address (not included as a Patient portal field selection)
  • Home Phone (not included as a Patient portal field selection)
  • Work Phone (not included as a Patient portal field selection)
  • Text to Cell
  • Fax (not included as a Patient portal field selection)
  • Home Email
  • Work Email
