About Well Visit: Anticipatory Guidance (AG)/Counseling

Path: Clinical tab > Patient Chart button > Well Visits > New or Open Note button > AG/Counseling

The Well Visit window charts a patient well-visit exam. The Well Visit window helps to code and complete a preventive encounter visit.  The well visit note encompasses both narrative and comprehensive formats. It also allows you to customize the layout of the note.  The note is added to the patient chart.

The AG/Counseling tab displays any counseling and anticipatory guidance notes.

Well Visit Function buttonsThe function buttons delete an entire well visit note, edit a well visit note, or save an well visit note.

Flag as Incomplete checkboxThe Flag as Incomplete checkbox will mark an encounter as unfinished.

Complete List button

The Complete List button lists the templates that are created, edited, and reviewed in the Well Visit Template Editor. Templates are pre-written scenarios you can use in a well visit to document common pediatric diagnoses. Templates include detailed descriptions for the usual pertinent positive and negative findings of a wide variety of illnesses, with a standard assessment, plan and ICD-10 coding. Included with your installation of Office Practicum are Pediatric specific templates.

Prior Notes buttonThe Prior Notes button opens a lists the previous preventive exam notes.

Anticipatory Guidance text box
The Anticipatory Guidance text box is where you can enter narrative note of the exam.  Phrase Construction can be used to assist with the creation of a Anticipatory Guidance or Counseling narrative.

Phrase Construction button
The Phrase Construction button opens the Phrase Construction window.  Phrase Construction assists with the creation of notes and other text-based descriptions.The Flag as Incomplete checkbox will mark a well visit as unfinished.

Counseling Text box
The Counseling text boxe is where you can enter narrative note of the exam.  Phrase Construction can be used to assist with the creation of a Anticipatory Guidance or Counseling narrative.

VisibilityThe Visibility dropdown selection determines who has permission to view the Counseling or Coordination of Care note. In the case where there is a critical sensitive matter (ex: abuse, high-profile) the note visibility can be set to an option higher than Any Staff Member.

The Well Visit Note Editor documents the patient preventive exam.  The Well Visit Note Editor contains the following tabs:

Care Plans tabThe Care Plans tab lists, prints, and manages care plans for a patient’s health maintenance schedule.

Visit DateThe Visit date field indicates the date that the patient preventive exam occurred.
