Objective 6: Coordination of Care Through Patient Engagement
Objective 6 requires a practice to use certified electronic health record technology (CEHRT) to engage patient/parent/guardian in care. An eligible professional must attest to all three measures and meet the threshold for two measures for this objective.
Objective 7: Health Information Exchange
Objective 7 requires a practice attest to all three measures and meet the threshold for two measures for this objective. If an eligible professional takes exclusion for all three measure they may be excluded from meeting this objective.
Objective 8: Public Health and Clinical Data Registry Reporting
Objective 8 requires a practice to attest the eligible professional is in active engagement with a public health agency or clinical data registry. The eligible professional must meet two of the five measures for this objective.
