Former Portal: Send a Form to the Patient Portal

Overview

Practices can make forms accessible to patients and parents on the patient portal. You will learn how to send Forms to the portal in this article.

The Patient Portal does not support users uploading forms, documents, or images for transmission to the practice.

If you want to have the parent access a Form from the Patient Portal, create the PDF of the form in OP 14 and send it to the portal on an individual patient basis.

Note: Prior to beginning the steps below, click the Print/Transmit/Save PDF button and select Save PDF in the smart toolbar.

Sending a Form to the Patient Portal

Follow the steps below to send a form to the Patient Portal:

  1. Select the Forms button on the smart toolbar.  
  2. Locate and select the patient.
  3. Select the Report from the Standards Reports tab or End User Reports tab that you would like to send to the portal.
  4. Select the Save to portal checkbox.

  1. Click the Print button in the Patient Medical Reports Manager (Forms) window.
  2. Complete the Disclosure Tracking window.

Viewing the Form on the Portal (What the Parent will See)

  1. Log into the OP Patient portal.
  2. Select the Patient for whom the form has been sent to the portal.
  3. Click the Patient Information tab.
  4. Click Forms from the Options section of the left side of the screen.

There is a  View link that opens the Form for parents to view or print. 

Note: Forms scanned into OP 14 can also be saved to the portal by selecting the Forms Item Type. For more information on Item Types, click here.
