Survey Editor

A list of surveys that can be imported into OP if they are not already available for you in your system.

Survey questions can be changed or deleted prior to using the survey for a patient. If the survey has been used on any patient, you must export the survey and import it back into your OP system.

To open the Survey Administrator Tool Bar: Click Utilities > Manage Clinical Features > Survey Administrator.

Once you have added a question to a survey, click the Answer button. The Answer Properties window displays.

After all questions have been created and assigned scores, you can create a Score Alert to have the system notify you of any results that fall above or below a certain value.

Exporting a Survey. Open the Survey Administration Tool Bar. Click the button in the Survey Administration toolbar. The Survey Forms Explorer will open.

To import a survey into OP the Survey it must have the .qst file extension. Click here to view the available list of surveys to download.
